Chart Deficiency Analyst


 
  • Job Objective: A brief overview of the position.
      Chart Deficiency Analyst position is responsible to review each discharged patient record for incomplete or missing items, identify the responsible practitioner for each of these items and link the deficiencies to the appropriate physician for completion.

      Education
      • Required: Highschool, GED or higher level degree
      Licensure/Certification
      • N/A
      Experience
      • Required: Two years healthcare or one year experience in a medical records department
  • Essential Responsibilities
    • Selects appropriate assignments for analysis from work queue
    • Reviews each record for missing documents, missing signatures or missing test according to procedure with 98% accuracy.
    • Assigns deficiencies to physician(s) according to procedure with 98% accuracy.
    • Links signature deficiencies for COLD fed documents to correct physician with 98% accuracy.
    • Correctly identifies when dictation has not been done and assigns to the correct physician with 98% accuracy.
    • Maintains analysis turnaround within required time frame at least 98% of time.
    • Is able to answer any questions physicians may have about their deficiencies and how to access assignments/complete deficiencies on IkeMD or directs to appropriate person for clarification.
    • Monitors reanalysis queue (physician decline queue) daily and reassigns deficiencies when appropriate. Maintains 98% accuracy.
    • Verifies physician dictation on the dictation system if physician indicates delinquent records have been dictated and dictated report is not in EPIC.
    • Provides daily reports to hospital personnel/departments on incomplete records and physician suspension according to procedure.
    • Track physician deficiencies. Utilize various communication tools to provide physicians with reminders
    • Prepares physician notification/suspension according to procedure.
    • Performs Quality Control Indexing when assigned by supervisor.
    • Performs routine clerical tasks and assigned projects by supervisor.

Schedule: Full Time, Day, 8 Hour Shift
